Prop Salvatore Perugini comes back into the team in place of veteran Racing Metro front-rower Andrea Lo Cicero while Canadian-born flanker Robert Barbieri reclaims his place from Paul Derbyshire.
In the backs Andrea Masi, who played at centre against Argentina before dropping to the bench against Australia, switches to right wing with Tommaso Benvenuti dropping to the bench.
Italy lost 22-16 to the Pumas in Verona before going down 32-14 to the Wallabies in Florence last week.
